About the Role: The Cityblock Virtual Hub delivers integrated primary and acute medical care, behavioral health and social care, powered by relationships and designed to advance equity in access and outcome. By delivering these acute and longitudinal services in-home through a combination of digital and in-person connections, we can allow members to stay at home and out of the hospital. As an additional service in support of our local Hubs, we provide trusted, quick, and convenient wrap around front desk support like scheduling, consenting, initial assessments, and more. I n this role, you will provide direct service to Cityblock members as part of our innovative care model, designed to address the complex health and social challenges of high-risk, high-need populations living in urban neighborhoods. It is integral to our care model that we meet people where they are, both physically and emotionally. This role will serve as a foundational member of our Connected Care contact center team. We are seeking an experienced RN who is able to jump in and set the example for future team members. As a Virtual Primary Care RN, you will work through text, telephone, and video to manage a panel of members to meet their urgent care needs and improve their health holistically. You will receive new diagnoses, manage chronic illnesses and help members facing an acute symptom, condition or medication related concern. You will receive inbound calls and are responsible for clinical triage. Using an electronic platform and assessment tool, you will conduct holistic assessments, care planning and leverage an interdisciplinary team to assist members to meet care goals. You will provide a necessary bridge to other care team members, facilities and resources. Additionally, you will provide clinical support to our large team of non-clinical team members including Virtual Member Advocates. Requirements for the Role: Have a current, unrestricted RN license in New York, or be willing to be licensed in New York Have 3+ years of experience providing clinical services to a variety of populations Have recent outpatient or telephonic case management experience (prefer 1+ year) Are willing to work 10-12 hour evening or weekend shifts (Ex. Sunday/Saturday 8am-8pm ET) to accommodate extended hours provided to our Cityblock members Have experience as an active participant in continuous quality improvement projects within a provider setting Possess exceptional triage, coordination and clinical assessment skills Are a phenomenal communicator; you approach care interactions with warmth and thoughtfulness Demonstrate the ability to affect change, and have been effective in helping a member or patient adapt new habits, or change behaviors Tech savvy, able to talk, type, and chart in the moment Ability to thrive in fast paced- startup environments Excited about how technology can support your work and help drive the ongoing evaluation toward new and better care An independent self-starter, a leader, and a strategic thinker who is excited about the big picture of whole community health, and the ongoing evaluation and iteration of our care model Believe deeply in a localized, community-based care model and are compelled by the mission that drives Cityblock Health Possess strong emotional intelligence and have the ability to influence Have the ability to effectively collaborate and execute in a highly matrixed environment Can excel operating with change and uncertainty, with a history of meeting or exceeding targets How We Define Success: Support a panel of adult and geriatric members to provide nursing clinical support/triage, including transitional care, health maintenance, medication reconciliation, chronic disease management and clinical support for members. Create, update and manage text/telephonic/virtual holistic assessments, care plans, goals and drive interventions that map to positive cost/quality outcomes, exceptional member experiences and exceptional care. Work daily within our custom-built care facilitation platform, which will enable you to collect data, organize information, track tasks, and communicate with staff, members, and family. Foster lasting and trusting relationships and leverage motivational interviewing techniques paired with strong clinical acumen to assist members in achieving goals, identifying new needs, and coordinating care Utilize critical thinking skills and excellent communication skills to manage complex clinical issues utilizing assessment skills and protocols Leverage strong time management skills to ensure that we make the most impactful judgement calls as we care for a full panel of members, and juggle competing priorities throughout the day Go above and beyond to connect with members and partners in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day Be responsible for managing an active caseload, be willing to perform outreach calls, receive inbound triage calls, and work as an active part of an interdisciplinary care team.
