Overview:The Regional Enrollment Specialist serves as the entry point for obtaining care management supports from Care Design New York (CDNY).

The Enrollment Specialist ensures an expedient entry into services and a seamless hand off to care management staff upon enrollment.

Responsibilities include, but are not limited to, assisting with eligibility, gathering documentation, interfacing with the NYS Office for People with Developmental Disabilities (OPWDD)Front Door staff, acting as a liaison with hospitals, and other service providers who may be seeking to refer individuals for services, and communicating with regional Care Management staff regarding incoming members.It is critical that the Regional Enrollment Specialist understand and incorporate into their approach to their work the following driving forces behind Care Design New York’s organizational culture:Individuals and families are at the center of all we do.We work for individuals and families.We value what individuals and families have to say.We are focused on outcomes that meet individual needs.We will be strong advocates and protect individuals’ rights.Responsibilities:Receive calls from prospective members, families, advocates, and professionals reaching out to inquire about accessing Care Management and other support services available through OPWDD.Perform enrollment activities in a culturally competent manner, considering a member’s needs concerning language, disability access, and limitations.Refer and assist members identify/schedule/obtain needed evaluations when necessary.Ensure a seamless, complete, and effective enrollment process for all prospective members.Manage an active caseload with prospective members in various stages of enrollment, until the enrollment process is complete for each.Perform needed data entry, maintain enrollment activity notes, upload supporting documents, and initiate the OPWDD eligibility processes.Assess and determine the need for OPWDD eligibility; assist those who do notyet have OPWDD eligibility in navigating the process to obtain OPWDD eligibility.Collect relevant information and assessments required for OPWDD eligibility and CDNY enrollment; submit to OPWDD for review.Review and complete psychosocial updates for potential members.Collaborate with OPWDD to complete the Level of Care Eligibility Determination (LCED) for individuals wishing to enroll in either Health Home Care Management or HCBS Basic Plan Support.Work closely with OPWDD Front Door staff to ensure all requirements are met for eligibility.Responsible for obtaining compliant DOH, OPWDD and CDNY consents as appropriate.Remain current in changing OPWDD policies and procedures to ensure that CDNY remains responsive in supporting people’s entry into services.Collaborate with CDNY Benefits & Entitlements departmentto ensure appropriate Medicaid eligibility for enrollment.Work closely with regional leadership to ensure a smooth transition to an appropriate Care Manager once approved by OPWDD.Remain knowledgeable about community resources and make interim support services referrals when a member/family is in crisis.Represent CDNY at regional information fairs and events, as necessary.Meet all training requirements on time.The enrollment position is currently remote, but subject to change dependent on organizational needs.Meet face to face with families when needed at one of CDNY’s Office Hubs.Must be able to travel and adhere to CDNY’s travel policies.Qualifications:Bachelor’s Degree and two years’ experience working with people with IDD(preferred) or Associates degree with four years of IDD experience.Proficiency in the use of internet-based applications, Chrome, and Outlook.

Must be comfortable navigating computer-based documentation and electronic timekeeping and training.Ability to learn all required computer applications.Proficiency with health-related computer applications.Reliable transportation is required, and a valid driver’s license may be required depending on location.
